The Modern Family star, who secured a temporary restraining order against Matthew Prokop this week after claiming he has been harassing her and abused her before their split after five years together last month, has revealed he sought treatment at an undisclosed facility after moving out of their Los Angeles home in mid-August.

The 23-year-old stated in court documents, obtained by PEOPLE.com, that she was advised to obtain the protection order by the director of the rehab facility before the 24-year-old actor was discharged on Sunday (21.09.14), who warned her about Matthew's "mental state and attitude."

In a slightly bizarre move (for those who are avid fans of the show - imagine), but one which total makes sense; the Emmy Award winner invited her on-screen mam Julie Bowen to their home to stage an intervention to help her break-up with Matthew last month.

The aim was for Julie to ''peacefully end the relationship,'' but Hyland says Matthew freaked out when he arrived home to an intervention. After he threw a lighter at her, Sarah claims Julie stepped in and told her to leave the house, as she wasn't safe around the actor.

She also says Matthew threatened to set one of her homes on fire and told her she would never see her dog again. To add to that, a mutual friend told her Matthew once said: "I deserve to die," while court documents obtained by TMZ say allege Matthew pinned her against a car during an argument about her outfit in May before repeatedly calling her a ''c**t'' and choking her.  You may also recall that he went a bit OTT on twitter after that incident in Australia involving the groping.

In a statement released last night, Sarah's attorney, Lee A. Sherman, said: "Out of respect for the court, the process and all parties, I have advised Ms. Hyland not to comment on the matter."
