The 28-year-old actor, who has sparked concerns over his mental health in recent months, is allegedly seeking treatment at a private facility in Los Angeles.
A photographer from X17.com said: "Shia was nervous; he didn't look good all morning. He was looking down and wasn't even talking to his driver. It looked like he didn't want to do it [rehab], but he knew he had to."
The news that the Transformers star is seeking professional help comes just days after he was arrested during a Broadway performance of Cabaret in New York City last Thursday.
Shia was charged with harassment, disorderly conduct and criminal trespassing after he began yelling offensive language at security guards and slapping people's bottoms midway through the show at Studio 54.
It is also believed he struck up a cigarette in the theatre and then shouted obscenities at police officers as they tried to handcuff him. He's expected to appear in court in relation to the charges on July 24th.
Meanwhile, Shia's friends and family became increasingly worried about the star earlier this year after he stormed out of that Nymphomaniac press conference in Berlin, before arriving on the red carpet for the film's German premiere wearing the now very famous paper bag on his head. Then there was the sky written apology for the plagiarism, the teary art installation, the transatlantic headbutting and so on (namely chasing homeless guys around New York).
